Patient cleared and accepted on unit via admission from local area ED. Access TeleCare clinician psychiatric mental health nurse practitioner (PMHNP) or physician performs initial assessment and medication evaluation with 24/7/365 rapid response coverage, streamlining the process.

Daily Rounding

Patient is seen daily by Access TeleCare PMHNP during rounding and as need via on-call coverage. Access TeleCare MD rounds once or twice per week as well.

Establish Care Plan

Access TeleCare provider establishes and executes a care plan including medication management and psychotherapy as needed.

Clinical Collaboration

Access TeleCare clinician documents all visit notes in your EMR system including any medication recommendations. Access TeleCare MD provides supervisory medical directorship and holds weekly Interdisciplinary Team Meetings with PMHNPs.

Discharge & Billing

Access TeleCare clinician will complete discharge summaries within 24-48 hours. Access TeleCare platform collects needed patient info from hospital EMR and clinician notes and passes to RCM for billing.
